Personal Care Program
Provides an In-Home Aide for a maximum of 60 Hours per Month.
Information:
The Personal Care Services provides a limited number of hours of In-Home Aide assistance to help individuals remain at home and maintain health and dignity. Your Physician must refer you to an idependant assessor in Raleigh. once the referal has been recieved, an nurse will visit you for an assessment and the hours you recieve will be determined by that visit.
Services:
Services include bathing, dressing, and toileting, and meal preparation.
Eligibility:
To be eligible you must be eligible for regular Medicaid, and have a medical condition that requires direct on going care prescribed by a physician. You must need help with personal care tasks due to a medical condition, and be medically stable.
