Managing Life Enhancement Services
Even college degrees don't prepare you for managing a mature life.
At a time when life may have grown a little more difficult and faculties somewhat impaired, older adults find they must master a whole new group of support services. Often these services must be obtained from different agencies with different requirements and vocabularies.
Professional Support
By providing our clients a Case Manager to assist them, we ensure that a knowledgeable professional provides individual support by planning, coordinating, and arranging for enrollment in the programs and services that best meet each client's needs.
Nutrition Services
Proper nutrition and a well-balanced diet are necessities of life. For those who are homebound and unable to prepare their own meals, we offer the Meals on Wheels Home Delivered Meals program throughout the county. For those individuals interested in socialization and participating in various activities, we offer Group Meals at both of our senior centers.
PAS-TRAN
Convenient transportation ensures that our clients are always able to travel to their appointments. Typical requests are for transportation to medical offices, pharmacies, stores, service agencies, as well as to our own senior centers.
Home Delivered Meals
Applicants for our Meals on Wheels Program must be a resident of Pender County, age 60 or older. A phone screening assessment is required to determine need, and eligibility for the delivered meals program.
Group Meals
Socialize and have lunch at Heritage Place, in Burgaw, and at the Topsail Senior Center, in Hampstead. Some clients may desire transportation to and from the Congregate Meals Program.
Lunch Hour: 12:00 - 1:00 pm
